Calm Harbor is a community-based residential care facility that supports an adult mental health population who have recently experienced a crisis event in their life. These individuals are not in need of ongoing medical care but do require monitoring and support. The facility is located in West Bend.
The Adult Care Worker is responsible for implementing the post-crisis case plan objectives and strategies for each resident in a manner consistent with standards of care established by State and Federal regulations and the policies and procedures of Elevate, Inc..
